Every effort will be made to accommodate your preferred time.

Despite our best intentions, we sometimes run late! This is because someone has needed unexpected urgent attention. Our reception staff will attempt to contact you if there is an unreasonable, unforeseen delay with your appointment.

Please advise if you require a Longer Appointment. Complex health issues or emotional difficulties may require extra time with your doctor. Longer appointments should also be booked for a pre-employment or insurance medical. If more than one member of your family wishes to see the doctor at the same time, please ensure that a separate appointment is made for each member.

Urgent cases will be seen on the day.

INTERPRETER SERVICE:

If you require an interpreter service, we can organise this for you; please let us know when you make the appointment.

CARE OUTSIDE NORMAL OPENING HOURS:

This clinic does not provide after-hours care. In the case of emergency, please phone Cobdenhealth directly on (03) 5595 3100. The nursing staff will contact the on-call doctor if available or refer to relevant emergency care centre or encourage you to call “000”.

If you are seen at Cobdenhealth in an afterhours or emergency situation by our on-call doctor, this service will be provided privately. You will receive an account for attendance. You will receive a Medicare rebate, however there will be a gap payment required. If you require medication, the Cobden CarePlus Pharmacy will send you an account.

HOME & OTHER VISITS:

Home visits are available (within the Cobden Township) for regular patients of this practice whose condition prevents them from attending the surgery. To request a home visit, please call the clinic first thing in the morning.

TELEPHONE ACCESS:

In the interest of patient confidentiality, doctors will not receive calls whilst consulting. If you wish to speak directly to your doctor, please leave your contact details and request that the doctor return your call. The doctor will be advised of your request in due course and will contact you when free.

In the case of an emergency, the doctor will be advised immediately and will speak with you as soon as possible.

SERVICES AVAILABLE:

Apart from general medicine and chronic disease management, the clinic offers a wide range of services including minor surgery, counselling, employment & insurance medicals, accident & emergency treatment, spirometry, ECG, blood pressure monitoring and immediate INR estimation for Warfarin management etc.

REFERRALS

The doctors in this clinic will competently handle all of your common health problems. When necessary, your doctor will be able to draw on opinion from specialists and if need be, refer you for further investigations / treatment.

Fees and Billing Arrangements:

We are not a bulk billing practice.

Payment in full (or balance after Medicare) is requested on the day of consultation. We accept cash, cheque and EFTPOS.

Bulk billing applies only to:

• Veterans’ Affairs card holders

• Patients over 75 years of age

• Pension card & healthcare concession card holders

• Patients having routine blood tests

• Patients having routine immunisations

• Patients under 16 years of age

If you are having problems with these arrangements, please discuss the matter with your doctor.

Getting the results of any test or procedure:

Your doctor will advise when he or she expects the results to be available. Reception staff will contact you if an urgent or non-urgent appointment is required. You may call the practice to find out your results.

Reminder System:

Our practice is committed to preventive care. We may issue you with a reminder notice from time to time offering you preventive health services appropriate to your care. If you do not wish to be part of this system please advise your health care professional.

Management of your Personal Health Information (Your Privacy – Our Policy):

The provision of quality health care requires a doctor/patient relationship of trust and confidentiality, and consistent with our commitment to quality care, this practice has developed a policy to maintain security of personal health information at all times and to ensure that this information is only available to authorised members of staff. Third Party:

A suitably qualified third person, i.e. a Medical Student or Registrar, is occasionally present at consultations. On arrival, patients are advised of the presence of the third person and are at liberty to request that they leave the room.

Periodically, Cobden Clinic may participate in research and quality assurance activities, to improve your health and wellbeing, community health care and practice management. Usually de-identified data is used, but should information that will identify you be required, you will be informed and given the opportunity to “opt out” of any involvement. You may choose to opt out of the above at any time in the future by advising your GP, practice nurse or receptionist.

Your Rights:

If you are not satisfied with our service, please contact us. Please feel free to talk to your doctor or to our receptionists. You may prefer to write to us or to use our suggestion box. We take your concerns, suggestions and complaints seriously and aim to resolve them quickly and fairly.

If you remain dissatisfied with our response, you may contact the Health Complaints Commissioner (HCC). The HCC responds to complaints about health services and the handling of health information in Victoria.
