Home Care Packages

If you need support to remain in your own home, you may be eligible for a Government Subsidised Home Care Package (HCP).

A Home Care Package has full felxibility to tailor services to suit your needs. This model of care is called Consumer Directed Care, and offers an array of choices to support you at home.

Supports available to HCP clients include: nutrition, domestic (home cleaning) and personal assistance (showering), transportation, shopping assistance, nursing and allied health services, mobility, aids and equipment purchasing, and in-home respite, just to name a few...

The professional case managers at Cobdenhealth will help to plan and coordinate your in-home care services.

Eligibility

The Aged Care Assessment Service (ACAS) will determine your eligibility for a home care package, or other services to help you in your home.

To have an Aged Care Assessment, you must be aged 65 years and over, or 50 years and over if you identify as Aboriginal or Torres Strait Islander. Assessments can be done in the privacy of your own home, and they are free.

Assessment involves cognitive screening,evaluating the ability to manage day-to-day activities (showering, cooking, cleaning, shopping), general health, and discussing what you want, or need. The assessment is required to determine your eligibility for the appropriate level of care, and the outcome of your assessment will be communicated with you.

Case Management

You decide the level of involvement you want with managing your Home Care Package.

Our Case Managers meet with you to discuss your goals and preferences to develop your care plan.

Your package is tailored to your needs and has the flexibility to adjust as your needs change.

The fees payable vary depending on the level of services required and the outcome of your Centrelink Cost of Care Assessment. Fees are negotiable between Case Manager and Client, upon acceptance of a HCP.
